  • At least 18 years of age
  • Within range of treatment
  • Not pregnant or nursing

    • Collagen vascular, autoimmune (rheumatoid arthritis, Sjogren’s, lupus, etc.) or immunodeficiency disease
  • Evidence of keratoconus
  • Patients taking isotretinoin (Accutane) or amiodarone hydrochloride (Cordarone)
